@alexis there are several things at level up that can alter ac so its a good idea to check it. Gear is accounted for in the answer. And i added a note re: unarmored ac for when dms gank you in your sleep

@webbcode probably worth mentioning that swarms are not automatically insubstantial, but often resist half damage against melee and ranged attacks. They mean distinctly different thing. (insubstantial halves all damage)

@Korack if you wanted to make it on topic here you'd need to offer more criteria than "OP." You'd need to explain why the were templates in the MM don't do it for you, what you're trying to accomplish and overall some kind of objective measuring stick with which to judge what you're doing. Overall it's better to ask questions like "I'm trying to do something like this and am stuck on point X, help me get unstuck" than "critique my homebrew"

@PurpleVermont That's a separate question really, but check out the combat chapter under "Ranged attacks in close combat" ranged spell attacks are still ranged attacks.

This is probably too broad, what you're asking for is for us to develop home brew rules for you. What would probably be a better method for this would be to come up with your proposed solution and then ask us if we see any problems in implementing it. Basically you need to be further down the road of implementing this before it should be a question here.

@SevenSidedDie thief's reflexes gives you a second turn in a round, that pretty much breaks that logic.

@SevenSidedDie This really has to mean turn rather than round else other rogue features become way underpowered (see Thief's Reflexes for one example)

it does not specify once per your turn it's 1/turn which could indicate another creature's turn. If this answer was correct, Thief's reflexes would not allow for sneak attack on your second turn. Turn != round in this case.
