comment Can a character gain two levels with the same encounter?
And a Deck of many things also has a risk. The characters could have been reduced to a pile of goo with out a save. That is the nature of the deck. I would hate to nerf the best card in the deck just because your characters got lucky.

comment How to improve physical attractiveness of a PC with low Charisma?
@SevenSidedDie - That could be I started with a group that allowed it and so did most of my friends. When 2ed came out we stopped using it anyway. There is also no rule that I know of that allows for altering stats 1-1 but it was a fairly common house rule as well.

In AD&D(v1) - Unearthed Arcana an ability of Comliness was introduced as an optional stat. It almost always was just the place to stick your lowest ability score because it really meant nothing. In campaigns where the DM allowed adjustment of scores (1-1 or 2-1) it was almost always lowered to the lowest number allowed.

comment How would I simulate the multi stage fitness test?
I guess I am disappointed that this SE has decided that since this is not an easy question to answer we are going to put it on hold. The question is clear. I want to implement a challenge that is a common fitness test, how can I do this in a way that is going to be fun and more than just a series of seemingly endless die rolls.

comment How would I simulate the multi stage fitness test?
@GMJoe - I think a good answer would actually explain all of that. The question is how would you perform the tests and come up with a result. I think the answer would present a solution that minimizes the number of actual rolls while giving still a decent result.

comment Tried all the existing advice, but players still never roleplay fear
We had a house rule that if PC died in combat the group only got half xp for the night.

comment Can a lich be controlled by control undead?
I think that some of the defenses you are suggesting the Lich would use would actually go against the spell. Specifically casting teleport or silence to prevent the controller from giving orders and attacking allies when it has been commanded to stop. It will protect its phylactery much like you can not command it to kill itself. You control the lich's actions not its knowledge. A lich will probably cooperate if it is being used as a weapon against others(particularly living creatures) but resist being used as a tool for self advancement.
