I believe that Blackmoor is part of the Mystara setting in TSR Products. There's a 3rd party book on the original Blackmoor setting, which is different, and I believe portions of it are included in Greyhawk as well.

What parts of the geographic change are "in world changes" vs "out world changes"? I recall WOTC saying that the map was condensed to have more land and less sea, but that this was not a physical change -- the world has retroactively always had the new shape.

There are two theories of adoption -- one is to find a game very close to traditional, so there are fewer barriers to adoption. The other is to find a game as far from traditional as possible, so you aren't tempted to fall back on old habits. Which are you looking for?
