Very minor nitpick: Kobolds are as smart as humans in 3.5e; Their racial intelligence modifier is 0. I'm pretty sure their Wisdom and Charisma modifiers are 0 or better, too, though I don't have my Monster Manual at hand to confirm it.

@HeyICanChan Most of them are from the end of the adventure "Die, Vecna, Die!" in which the titular power managed to upset the multiverse before being defeated. Unfortunately, there was never any detail published that described which inner plane ran aground on which prime world, which outer plane drifted away, and what the consequences were, so there's not much of use that I could add - except that it's a popular fan theory that it explains the differences in the cosmology between 2e and 3e. Given that it's D&D lore and not PF lore, it wouldn't be that useful anyway.

@Zibbobz The Mongoose Traveller core book contains a set of rules for point buy, and I believe the GURPS Traveller version uses GURPS-style character generation. I tend to find the semi-random method more fun, though.

+1 for letting spell components be an element that drives the narrative. Running out of eggshells is an easily solvable problem in a farming hamlet; Running out in the depths of the Abyss is another matter altogether.

@Jim He has 1 rank in swim and a +3 bonus for it being a trained class skill. A +3 bonus is not +3 ranks; It's a +3 to the result when you make checks with the skill. His strength modifier applies a further bonus. The 3.X family of games is all about stacking different types of modifier; Skill rank is just one of these modifiers.

This isn't enough for a full answer, but players who break immersion often do so deliberately, to keep things from getting too tense. Often, such players are more than willing to hold back for a session if you ask them, and explain that you want to maximize the tension and emotional impact of the session.
