@MarcDingena We use the facing of the player making the roll. That said, it doesn't really matter how you determine which is left and which is right as long as it's unambiguous whenever a roll is made.

In a film, the first (and zombie-free) act exists to set up the "normal lives" of the characters, and establish who they are outside of the horrible, horrible situations they later find themselves in. Sure, you can learn a lot about someone by seeing how they act in horrible, horrible situations, but unless you can compare that with how they live their lives normally, you'll not fully understand how those horrible, horrible situations change them.

I'd be weirded out if I met Vecna and he was carrying the hand of Vecna. I'd be all "Wait, you're a lesser deity, at least - Can't you just grow a new one? Besides, isn't your whole thing leaving your spare organs around to tempt people with? Has the world run out of corruptible wizards or something?" And then he'd say "No, it's just that you're next wizard on the list," and I'd be like, "Oh. Ooh. Um. Is this optional?" and then it'd be awkward.

@Mala Your players have decided that they want to have high armour classes instead of another feat. How is that more of a mechanical problem then their deciding to know magic missile instead of burning hands? What do you think the problem is, and why does this answer does not solve it?

My group takes the "back-of-party mode" a step further: If a player's not present, we treat their character's location and activities as indeterminate, and let the absent player specify whether they were with the group the whole time or elsewhere doing something else when they come back. It works pretty well, though we've yet to test what happens when the present PCs get TPK'd.

Do not underestimate how important these features are to the game! Back when I was in a M&M game, we made the mistake of trying to codify every ability we'd ever have as a power, but the superhero genre is all about coming up for creative uses of the powers you already have - and these rules are what support that.
