comment If a DM uses Zone of Truth as a permanent effect, should creatures get only one save?
if you are particularly simulationist, you might want to have several difficulties based on the type of lie. Lies of omission being easier than total falsehoods. White lies might be the easiest, leading to instances where players accidentally tell the temple's leader that they think their hat is silly.

@Zachiel I assumed all of the effects were listed in CatLord's question.

comment When giving out magic item treasure, do I replace or supplement what characters have?
This problem with 4E has lead to me just handing out whatever-the-crap and letting them hock/disenchant the things in search of stuff they really want. Also, the players read through the books and tell me things they want, and we make those secondary goals on their quests. As well as creating my own iconic items for characters specifically as you mentioned.

comment Having player characters in different locations in a city
A note on combat: I have had success in the past engineering it so that a split party all ends up in combat at the same time in different locations. You can then run it as if it is all one battle.

comment How do I prevent my players from immediately recognising an ambush?
I make it a habit to ask my players to hand me their passive perception or insight scores at random times, just to keep them on their toes. Also "How much HP do you have, again?"

comment Can a lich's phylactery be recreated?
Not sure about 3.5 but the 4E Archlich Epic Destiny text explicitly talks about creating a new Phylactery should your existing one be destroyed. It takes 10 days and costs 50,000 gold.

comment Do reading spectacles allow one to understand all languages, not just read them?
This is my understanding as well. And this fact is what lead me to not only writing a book in Drow, but to encoding it with a substitution cipher. Just because they can read the words, doesn't mean the words make any sense. :)

Arts&Crafts stores like Michael's and Hobby Lobby carry great potion bottles.

Try getting the Emblem of Ossandrya and really really pissing it off for a -2 to savings throws.

however, nothing stops you from paying the full cost of the ritual and flavoring it as a refill.

comment D&D 4e Campaigns/Adventures that spell things out for a brand new DM
In many ways it is easier to run a few one-session mini adventures of your own design. Just pick a setting, describe a place in it that the characters are, pick a few monsters out of the manual following the DMG rules, and keep it simple until you get the swing of making stuff up on the fly. (yes... You CAN push that statue over on the goblins.. Role Strength... Uh... It crushes one of the goblins flat. SQUISH!!!)

comment What can I use for sci-fi ambient music?
Actually, I created a bunch of Spotify playlists for Shadowrun. This is the all list. There are playlists for Astral, Sprawl, and the Matrix and many others.

comment Does D&D 3.5 DM material contain information applicable for Pathfinder GMs?
Generally speaking, all DM/GM guides contain practical advice that would be beneficial to all DMs/GMs of any game. My advice would be to read as many of them as you can.

One place to go for getting close to this are the Phoenix Wright series of games. They deal with uncovering clues in a similar way to how you'll be having your kids do with the above method.

comment Who should be addressed in an RPG rule text?
Actually, in the example cases, it is clearer and more concise when not consistent. "A Psychic Warrior calls forth the aggressive nature of the beast inherent in him/herself, psionically transforming his/her hands into deadly claws."
