This seems mostly good, but there is a difference between a license and a contract. The GPL, for instance, is not a contract. I'm not sure about the OGL, though I suspect it is also a license. (What it's named isn't what matters.)

@Aaron In AD&D edition they got both druid and wizard spells, so maybe it was a lingering memory of that. :)

I suspect many of the game designers did treat immediate actions as "interrupts"; there are some abilities that make little sense otherwise.

Ok, but then more than half your post is a distraction from that point. Just cut out the middle part and leave the (important) idea that your ability to remain in combat equals your Armor Class + Hit Points.

I actually just noticed that 5e switched the generic terminology to "known in mind", to cover both prepared and spontaneous casters. At least, that's what they use in the chapter on magic.

Sidenote: divine casters don't need a free hand to use their holy symbol as a focus: "To use the symbol in this way, the caster must hold it in hand, wear it visibly, or bear it on a shield." Somatic gestures are still an issue.

@NiteCyper A fair number of 3.5 materials got very limited or no errata. (They were weirdly reluctant to issue errata for that edition; lots of very obvious mistakes were never officially corrected in that way.)

@Tridus In PF, you don't even move into the square of the creature you're grappling; conceptually, it's not as up-close as it was in 3.5.

@SevenSidedDie obv I don't want to get into a huge old discussion in the comments (and for personal reason I won't use chat), but my whole point is about what, actually is "perceivable". I'm saying that there are some things that seem abstract and gamey that our characters would perceive in a very concrete way. Abstracting something out into a mechanic (HP, initiative order, spell levels) doesn't mean a character doesn't know about it; it's just they would describe their understanding in non-gamey terms.

I feel like the idea of initiative is fundamentally gamey enough that anything related to it will likewise feel gamey. Your character already knows the exact order that folk will act in -- it's not clear from the rules how, but they do, in some way, "perceive" it. What does it mean for your turn to come up? Presumably it's some window in the general flow of combat that allows you to act uninterrupted; I don't see a reason not to ready an action for that same window of opportunity.

"Small elemental are not the brightest kids around" -- in a vaguely similar circumstance, my group simply buffed the elemental with fox's cunning and comprehend languages. Luckily this combo still only requires 3rd level casters.

@SevenSidedDie stone salve is a thing. I mean, it's not a potion in game terms, but it is in more casual usage. (And from what I've seen of 5e, would probably be called a potion there.)

@shaydwyrm You could always use that same logic to argue that a bat really has blindsight, and not blindsense. They can track the position/velocity/location of small insects, and notice obstacles with the resolution of a single hair. So if you go by the RAW, porpoises; if what makes sense, argue for bats. :)
