Marco's 3.5 7-PCs-campaign - aborted for scheduling problems. Still no level ups.
Marco's intro to RttToEE - gave DMing right to me after 2 sessions. Arbitrary leveling up between those.
My 3.5 Return to the Temple of Elemental Evil. TPK'ed at level 8/9.
Marco's 3.5 all manuals campaign - aborted for excessive PC firepower. Guess what.
Marco's 3.5 premade characters campaign - aborted after less than 10 minutes of play.
Pierluigi's 4e Keep on the Shadowfell - gave DMing rights to me before I could level.

I think you're reading that rule wrong. You may just means "if you want to, that's the maximum you can get". The fact that you can does not imply that you must.

I wouldn't say the dead levels are there for "balance". Removing a dead level from a fighter and putting some useless class feature wouldn't change much in the fighter balance and wouldn't balance it against, say, a wizard. They're there for keeping a paced progression of class features. If a class had sneak attack every 3 levels and a feat every 2, there would be levels where the character gets nothing and levels where he gets both a sneak attack dice and a feat.

How essential is it that you're not a divine caster? There's a class that pretty much plays as a wizard: int-based spells, memorized daily from a spellbook and learned from scrolls. It's a divine spellcaster, but it's not as tied to religion or devotion as a Cleric/Druid could be, and I could write an answer on it if it's ok for your prereqs.

@ElSuscriptorJusticiero He was being attacked by a frenzied berserker right after their target dropped dead. Three high rolls, and he was at -10 with the second. Since the enemy was already dead and the encounter was ended we ruled a level up that could have saved him if the second hit just made him fall unconscious. By rolling max, he went to exactly 0 hp with the second hit and was felled by the third.

I'm not the downvoter, but keep in mid the tier system does not only talk about combat. It speaks of versatility and being able to solve many different situations.

It's the full "round" an action takes to complete. Which means all the 6 seconds, but not out of his turn (you can do a full round action, then do a swift action). They should have called it a "full turn" action.

In reality you get a little more thaan 50%. Just to show you, let's work with two failure dice and look at the probabilities: 1+1 1+2 2+1 1+3 3+1 2+2 are the no results. 3+2 2+3 3+3 4+1 1+4 4+4 4+3 3+4 2+4 4+2 are the ok results. It's 5/8 and I guess it grows when you have more failure dice to combine togheter since you can choose the best combinations, plus you can also sum 3 or 4 low dice into a success.
