In my spare time, I tell my daughter stories about Batman, read books, watch movies, play video games, and try desperately to play D&D even though nobody I know wants to.

This was my approach for a few years, and it worked very well. Sometimes we'd use a whiteboard so we could move the markings around, but we only did it once in a while, not constantly, and there was no measurement involved.

We have the same rule, but instead of a bell, someone will just "call bullshit." When bullshit is called, play resumes. If only one guy is pathetically calling it all the time, the game is potentially doomed, and the best thing to do is to have a real conversation about it. (Obviously, this is not for every group.)

comment How do you handle large scale exploration?
Given the number of mentions people have made about "aerial view" and "geographical map" it seems worth noting that you could easily turn the concept map (mentioned in my answer) into a hybrid of geography and concept. Then the party can pick starting points (we go to the "visible from space parking garage") and you can know where they might end up when they say "we wander around the neighborhood."

Delegating rules look-ups has saved me quite a lot of time. Tom is my rules guy, and he has a copy of PHB (and now Compendium) with lots of flags for relevant sections. It's probably the best time-saver of all the delegation I've done.
