-1 If it's in the rules, and a fairly standard tactic (you can't honestly say that hiding in a bush is an exotic tactic, can you?), and the players expect it to work a certain way for them, then having different rules for NPCs would be the abuse of DM power. And simply retconning situations where the PCs act stupidly is something I personally find horrible. Take the dice as they fall, and turn it into a story. And if the players can't handle it, they should be playing a different game. D&D is about killing things while they try and kill you, and if you can't take that, change system.

I'm not sure how to feel about this answer - I don't think that saying 'no-one can ever die unless they do something very risky/another player kills them' will improve a game, since everyone will be complacent about all 'normal' encounters. However, I agree with your point about Player B's bad luck killing Player A being a less-desirable outcome.
