(very old answer, new comment) I typically give the original weapon expertise feats for free (and allow players to take as many are as necessary to cover all weapons, implements and unarmed), but do not allow them to have the newer essentials feats for free because they add a powerful rider (e.g. "staff expertise" - not to be confused with "weapon expertise (staff)" also gives you the "don't grant combat advantage for ranged powers" effect). Having now played (and DM'd) at epic for a while, I'm not really convinced they're required anymore, but missing attacks is very frustrating.

Thinking about it, with regards to point 4, I think it would be better to allow the rider to confer training (if the rider has such training) in the skill, rather than allowing usage of the rider's skill modifier - there's a reason why clumsy fighters in platemail don't have a good stealth - but the rider could at least improve the situation. What do you guys think?

I don't think I would allow the mount to move more than once per round, though either character could initiate that move. Whichever one didn't move the stack still has a move action, they just can't use it to move (unless it's the rider dismounting)

I agree it's not directly answering the question, however I added it since, in my experience, whenever someone is asking about forced movement and teleport it's because they want to teleport an enemy somewhere it doesn't want to be.

Yes I did miss the explicit monster manual 1 reference; I should probably have added that the monster manual 1 monsters are fine, in my experience, but that monster vault ones need to be used with caution until later levels.
