This does seem like a logical parsing of the rules, but I have a hard time believing that it is intended. (I suspect it's just a side-effect of attempting to write the rules in natural language instead of as a technical code.)

Unless the disgruntled customer punches you, you shouldn't need to make a concentration check, as "normal activity, such as moving and attacking, doesn't interfere with concentration". The PH says that the DM may call for a concentration check due to environmental conditions, and gives the example of a wave crashing over you in a storm. By contrast, being yelled at by an irate NPC is all in a normal day's work for an adventurer.

I think this significantly differs by edition. I don't know about Pathfinder, and have just started with 5e, but one 3.x answer is to use gestalt classes

Another key point is how it affects critical hits: when you have advantage, the chance of getting a crit (almost) doubles. Conversely, when you have disadvantage, it becomes tiny. And of course, the situations are reversed for critical failures: incredibly rare when you have advantage, quite a bit more common when you have disadvantage.

Also remember that this is a party game: one player shouldn't have all of the skills. Rely on your companions (and let them shine). If you are playing a game with only one or two PCs, asking the DM for additional skills might be a reasonable way to help compensate.

Appealing to fantasy sense rather than RAW... a druid incanting while gesturing with a totem of feather and bone, a sorcerer doing the same with a glowing crystal, or a cleric tracing a pattern in the air with her holy symbol... all of these seem perfectly right. Doing the same with a shield painted with a symbol, on the other hand, seems a little off.

For what it's worth, a fast pace of advancement within a campaign can lead to two problems: 1) it makes it hard for the DM to plan ahead, since characters may be way beyond a planned challenge by the time they get to it, and 2) it can create cognitive dissonance, because serious NPC threats you met "yesterday" become way beneath you. It's a bit weird to go from from fighting rats to fighting demigods in under a month of in-game time.
