Reach is an attribute of a entity. Normally it based on the creature's size. But it can otherwise if the entity wields a weapon with reach or has some other attribute (like long tentacles) that give is a longer reach for its size. Attacks of opportunity work within a entity's reach period. Finally what Polearm master does, among other things, is grant opportunity attacks when creatures ENTERs your reach. This is a benefit on top of the existing rule as it does not normally happen.

comment Does 5e address the murderhobos problem?
Murderhobo has a history of being a loaded term that is used by many to criticize what many perceive the stereotypical playstyle of past edition. As with all stereotypes while it has some truth it is near useless in judging the specific of a situation or how a individual plays. I bring this up not to criticize its use but to remind people not to color their judgement when reading 5e adventure. In the two released adventure I noticed a liberal use of defeat and similar terms that I believe tells the referee (and players) that there are more options than killing.
