Gaming background in D&D (Basic & 1st ed. AD&D), Traveller, Judge Dredd, Call of Cthulhu, Rolemaster but have played pretty much only Glorantha-based games this millenium, excepting games at cons and some other one-offs. I like reading rulebooks, though. I haven't really followed the D&D rules since 3.5e.

@mxyzplk - If you think option #3 is nothing to do with in-game fiction, then I'm not doing a good job of communicating. I happen to think that in-game fiction solutions can only partly resolve conflicts about what players want from the game, so you will usually need assistance from either game mechanics or gamer-to-gamer discussion. The difference between #2 and #3 is that #2 aims to solve problem mechanically on the gamist level and #3 on the narrativist/simulationist levels, perhaps with support with changed mechanics. Maybe the distinction isn't as clear as I hoped.

@BrianBallsun-Stanton: Yes: the book was called "Designers and Dragons", was brought out in October 2011 by Mongoose, weighed about 400 pages, and is now out of print. Appelcline is currently working on a revised and expanded 4-volume version to be published with Evil Hat: IIRC, the drafts of the first two volumes are with the publisher, and the other two drafts are nearing completion.

"None of these games have mechanical tie-ins to Honor or Valor" - Nothing with high production values, but, e.g., David Dunham's PenDragon Pass gives it a decent stab in a Ralian setting (barabarians meet knightly idealists). The RQ materials already stress cult virtues, and HQ2 has good mechanics for them. See poppyware.com/dunham/pdp. Reasonable cult virtues for Humakt (there are a few write-ups out there) might include Honourable, Truthful, Ruthless, Dispassionate, Courageous, and Understated - an OK character sketch for Pendragon.

Video 15 shows unrivetted chain, which as the chap say is below combat quality. I could make a video of my tinfoil plate armour. Another point: bodkins are the arrowhead of choice vs. chain, but perform relatively badly vs. plate. Hardened, barbed arrowheads are best vs. plate. Splinters - that makes sense: the beauty of plate is that blows that do not penetrate cause little harm. My impression of why plate is so much better than chain was because of swords and spears.

@DevSolar: Take a look at, e.g., youtube.com/watch?v=q-Xp56uVyxs - search for [longbow plate] for several others: this is an unhardened bodkin, better arrowheads vs. plate were widely used in C15th, according to the Royal Armouries. You say: "a bodkin tip slices through average chain with very little effort" - unhardened bodkin points can penetrate decent quality chain, but not easily. Do you think you could find a source for the Conquistadores claim?
