So far these additions have greatly increased the survivability of your monk as well as increased base damage by 1 size category, 4 monk levels (at the cost of 1 level), and also given you a decent STR buff for even more damage and to hit. http://dndtools.eu/classes/fist-of-the-forest/

You can further add on to here if so desired. So, for race you can use a Changeling with the Half-Minotaur template. This opens up another prestige class as well. The Warshaper: http://dndtools.eu/classes/warshaper/The Warshaper.

If you want to put a second level into this prestige class, you gain an additional +4 STR and +4 CON from Morphic BbodyBody. Thats +2 to hit, +2 to damage, +2 health a level, and +2 AC. Not bad. You can also add in a 3rd level to make your unarmed strikes have a range of 10 instead of 5 with Morphic Reach.

I know likely this is too late, but I'll make a go at it anyways.

Everyone else already covered it that tumble and jump are extremely important in order to bound about the battlefield. There are actually ways to make a monk do good damage in a fight and have decent AC.

+2 size category, +2 monk Levels for unarmed strikes, +2 to hit, +2 damage+2 size category, +2 monk Levels for unarmed strikes, +2 to hit, +2 damage

+2 size category+2 size category, +1 monk levels for unarmed strikes, +4 to hit, +4 damage

Defensive adjustments: Well, +1 monk levels for unarmed strikesthere are a CRAP ton here. It really varies per character and where you put your stats, +4but it will give you even MORE incentive to hit, +4 damagedump tons of points into CON. Sure you lose 1 AC due to size but +2 natural AC from the template and then your entire CON bonus to AC as well. Whats even better is that this AC applies to touch attacks AND when you are flat-footed. Pretty OP if you ask me.
