Your house rules seem like a reasonable compromise and add some interesting flavor to what could otherwise be a mathematical exercise. I'm also reminded of the AD&D rules: "If any creature reaches a state of -6 or greater negative hit points before being revived, this could indicate scarring or the loss of some member if you [the Dungeon Master] so choose" (DMG 82), which seems appropriate, given this edition.
